RESOLUTION NO. 09-7410
A RESOLUTION OF THE CITY COMMISSION OF THE
CITY OF OPA-LOCKA, FLORIDA, AUTHORIZING THE
CITY MANAGER TO MAKE PAYMENT TO FORMER
CITY MANAGER JANNIE R. BEVERLY IN THE NET
AMOUNT OF SEVENTY-EIGHT THOUSAND FOUR
HUNDRED AND TWO DOLLARS AND EIGHT CENTS
($78,402.08) AS SET FORTH IN EXHIBIT "A", PAYABLE
FROM ACCOUNT NUMBER 12-512110; FURTHER
AUTHORIZING PAYMENT OF THIRTEEN THOUSAND
SEVEN HUNDRED TWENTY-FOUR DOLLARS AND SIX
CENTS ($13,724.06) ON BEHALF OF JANNIE R.
BEVERLY, TO THE STATE FRS AND THIRD PARTY
PROVIDER FOR RETIREMENT AND DEFERRED
COMPENSATION BENEFITS, PAYABLE FROM
ACCOUNT NUMBER 12-512220; AND AUTHORIZING
PAYMENT OF THIRTEEN THOUSAND SEVEN
HUNDRED TWENTY DOLLARS AND TWENTY CENTS
($13,720.20) ON BEHALF OF JANNIE R. BEVERLY, TO
THIRD PARTY PROVIDERS FOR INSURANCE
BENEFITS, PAYABLE FROM ACCOUNT NUMBER 12-
512230
WHEREAS,Jannie R. Beverly ("Beverly") was a former employee and City Manager of
the City of Opa-locka("City"); and
WHEREAS, Beverly was the City Manager from November 15, 2000 and up until her
departure from City on December 10, 2008; and
WHEREAS, Beverly and the City entered into a written contract for employment dated
February 22, 2006; and
WHEREAS, the City Commission of the City of Opa-locka ("City Commission") desires
to authorize a payment for Beverly arising out of Beverly's employment with the City.
NOW, THEREFORE, BE IT DULY RESOLVED BY THE CITY COMMISSION
OF THE CITY OF OPA-LOCKA, FLORIDA:

Section 1. The recitals to the preamble are hereby incorporated by reference.
Section 2. The City Commission of the City of Opa-locka hereby authorizes payment
to former City Manager Jannie R. Beverly in the following amounts as set forth in Exhibit "A":
Net payment of Seventy-Eight Thousand Four Hundred and Two Dollars and Eight Cents
($78,402.08) to Jannie R. Beverly, as set forth in Exhibit "A", payable from Account No. 12-
512110; Thirteen Thousand Seven Hundred Twenty-Four Dollars and Six Cents ($13,724.06) on
Behalf Of Jannie R. Beverly, to the State FRS and third party provider for retirement and
deferred compensation benefits, payable from Account No. 12-512220; and Thirteen Thousand
Seven Hundred Twenty Dollars and Twenty Cents ($13,720.20) on behalf of Jannie R. Beverly,
to third party providers for insurance benefits, payable from Account No. 12-512230.
Section 3. This resolution shall take effect immediately upon adoption.
