SICA MEMORIAL DESIGN AND CONST~UCTION PHASE SERVICES WHEREAS, the Borough of Carteret is in need of design and constmction phase serVices in regard to the Peter J. Sica Memorial; and \VHEREAS, the Borough's Consulting Engineers, T&M Associates, possess the requisite skill and expertise for said services; and \VHEREAS, T&M Associates has submitted a proposal to provide said engineering services at a total cost not to exceed $6,000.00; and 'VHEREAS, the Mayor and Council have reviewed said proposal; WHEREAS, the Chief Financial Officer has certified that funds are or will be available for said purpose. NOW, THEREFORE, BE IT RESOLVED by the Mayor and Council of the Borough of Carteret, County of Middlesex, State of New Jersey, as follows: 1. That the proposal from T &M Associates for engineering services related to the Peter J. Sica Memorial Design and Construction Phase ServiceS at a total cost not to exceed $6,000.00 at the rate set forth ip T&M's existing contract with the Borough is hereby approved. 2: That annexed hereto is a copy of said proposal. 3. That tIlls action is approved as an award of a professional services contract pursuant to N.J.S.A. 40A:11-5(1)(a) in accordance with the Borough's Fair and Open Process as authorized by law. 4. That a copy of this resolution be forwarded to the Borough's Purchasing Department and T &M Associates. Sica Industrial Highway Memorial to be located at the Intersection ofIndustrial Road and Middlesex Avenue. This project includes professional services related to the improvements described as follows: Scone 01' Services We uilderstand that the Borough desires to eOl1stnlct a memorial in honor of fomler Mayor Peter J_ Sica at the intersection of Industrial Road and Middlesex A venuc within the existing large concrete traftic island. The small adjacent COncrete traffic island will also be improved to complcment the memorial. The improvements will c011sist of a eMU ground mounted sign, a three flag pole arrangement within a concrete pavcr arca, and be coinplemented by perimeter landscaping and mulch beds. Construction Phase Services Upon th" Borough entering into a fOl1na! contract modification with a contractor tc perfol1n the following project improvements, T&M Associates will provide contract administration and inspection phase services necessary to facilitate the improvements for the project. We wi!! monitor performance with the schedule and contracted costs and perfoml the required inspections and job meetings as necessary to dIscuss perfol1nance, any design and contract issues and upcoming tasks and plans for their implementation. Wo will produce and distribute minutes of all Ineeting$, Our staff will provide part-time constl11ction coverage Monday through Friday. As necessitated by tho contractor's work schedule, on-site inspectIon will be provided, as required, beyond an eight-hour workday and Saturdays. In the field, our staff will oversee the construction actIvity including performing and documenting inspections to detenuine if the work is done in general conformance with the contract plans and specifications. We will notify the contractor in writing of any unacceptable work or material, track the deficient condition and inspect the work lmtil it is corrected. We will address surrounding pl'Opelty and general public concerns that may arise dllrlng the eonstmctioll phase. We will work closely with the neighboring property owners to aohieve timely and efficiont'completion of the work with minin1al dismption and inconvenience to the residents, while maintaining a safe work zone. A single contmctor payment requisition will be processed by our offiCe. Pay Items will be checked lor accuracy with field measurements. We will manage the contract closeout prOcess, including execution of any affidavits, release of liens, production of maintenancc bond, warranties/guarantees, operations and maintenance manuals and other technical data, as required. Fee Compensation for our services as set fOlth above is proposed on an hourly basis, to be invoiocd monthly. Fees are ba:;ed on the rate schedule In our municipal engIneering contract.
