RESOLUTION NO. 10-8088
A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF
OPA-LOCKA,FLORIDA, APPROVING A CHANGE ORDER FOR
THE CONSTRUCTION CONTRACT FOR SHERBONDY VILLAGE
COMMUNITY CENTER,CONFIRMING THAT CBHC,LLC, HAS
AGREED TO UNDERTAKE RESPONSIBILITY FOR THE
CONSTRUCTION PHASE DESIGN SERVICES FOR THE
SHERBONDY VILLAGE COMMUNITY CENTER PROJECT,
INCLUDING ALL RESPONSIBLILITY FOR SUBMITTAL
REVIEW,RFI RESPONSES,COORDINATION OF REVISIONS TO
THE DESIGN DRAWINGS, INCORPORATION OF CHANGES
INTO THE DESIGN DRAWINGS AND FINAL REVISIONS AND AS
BUILT DRAWINGS,WITH THE CITY CONTRACT ALLOWANCE
OF $75,000.00 REALLOCATED TO PAY FOR THE SERVICES;
PROVIDING FOR INCORPORATION OF RECITALS;PROVIDING
FOR AN EFFECTIVE DATE.
WHEREAS, the current plans for Sherbondy Village Community Center require certain
revisions, both to adapt the building to a Silver Certified Green building, and to correct certain
deficiencies in the original plans; and
WHEREAS, the City of Opa-locka has terminated their agreement with the previous project
Architect; and
WHEREAS, the City of Opa-locka has retained CBHC,LLC,as contractor,to complete the
project; and
WHEREAS, CBHC, LLC has agreed to undertake the additional responsibility of
construction phase design services for the Sherbondy Village Community Center project,including
all responsibility for Submittal Review, RFI responses, coordination of revisions to the design

drawings, incorporation of changes into the design drawings and final revisions and as built
drawings, at a cost of Seventy Five Thousand Dollars ($75,000.00):
NOW,THEREFORE,BE IT DULY RESOLVED BY THE CITY COMMISSION OF
THE CITY OF OPA-LOCKA,FLORIDA:
Section 1. The recitals to the preamble herein are incorporated by reference.
Section 2. Change Order number 0001, submitted to the City of Opa-locka by CHBC,
LLC,is hereby approved,and CBHC,LLC,shall undertake responsibility for the construction phase
design services for this project, including all responsibility for Submittal Review, RFI responses,
coordination of revisions to the design drawings,incorporation of changes into the design drawings
and final revisions and as built drawings. The City contract allowance of$75,000 will be reallocated
to pay for these services. The City Manager is hereby authorized to execute Change Order No.
0001 in substantially the form attached hereto.
Section 3 This resolution shall take effect immediately upon adoption.

This Change Order will confirm that CBHC has agreed to undertake responsibility for the construction phase Architectural and Engineer
services for this Project,including all responsibility for Submittal Review,RFI responses,coordination of revisions to the drawings,
incorporation of changes into the drawings and final revisions and as built drawings. The City Allowance of$75,000.00 will be
reallocated to pay for these services.
It is our understanding that you have terminated your agreement with the project Architect who will be giving a release of the drawings so
that we may employ a successor design professional to undertake the responsibilities referenced above.Once you have the release and the
electronic files for the project drawings,please let us know so that we can get the successor architect involved.
The Original Contract Sum was $3,760,000.00
Net Change by Previously Authorized Requests and Changes $0.00
The Contract Sum Prior to This Change Order was $3,760,000.00
The Contract Sum Will Not be Changed $0.00

The Contract time will be increased by Days or will be determined at a later date
The compensation and completion time allowed by this Contract Modification
does not include any amounts for changes in the sequence of work,delays,
disruptions,rescheduling,extended overhead,acceleration,and/or impact cost,and
the right is expressly reserved to make claim for any and all of these and related
items of cost prior to final settlement of this Contract.
