Program: Wachusett Mountain Ski -Board Program
Who: Grades 6 - 12
When: Saturday Evenings
January 6th — February 10, 2018
(Meets once a week for 6 weeks)
Time: 4:00pm — 11:00pm
Where: Bus leaves Watertown High School parking lot at 4:00pm, SHARP
Fee: Register online with Wachusett Mountain on their website

Register online www.watertown-ma.gov for bus transportation. Fee is $150.00.
Please submit "Town of Watertown" waiver/insurance form (on Page 2).
Description: This program is designed to provide middle and high school students with a fun and safe atmosphere to
learn elements of skiing or boarding or to improve their skills. All participants must wear helmets while skiing or boarding.
Space is limited on the bus provided by the recreation department. Please register early. All participants' parents must
register online with Wachusett Mountain. Please register for the transportation with Watertown Recreation.
Registration is on first come first serve basis Maximum: 100 for transportation only

Please make transportation payments payable to "Town of Watertown." Please return registration to Recreation Office. Do not
return to the school department.

Wachusett Mountain Ski -Board Program
WAIVER
The signed parent/guardian gives (participant's name)
permission to participate the Recreation Department's Wachusett Mountain Ski -Board Trips. I
understand and agree to indemnify and hold harmless the Watertown Recreation Department, its
directors, chaperones or site owner from any injury and will assume responsibility for medical or
dental expenses incurred as a result of participation in this program. I also confirm that the participant
has personal medical insurance coverage and that any expenses incurred while at this program are
my responsibility. In case of an emergency, I understand that every attempt will be made to contact
the person listed. If contact is unsuccessful, I give permission to attending medical personnel to
render medical treatment to the participant. I also waive any and all claims for property losses against
the Watertown Recreation Department. I give permission to the Watertown Recreation Department to
transport my child to Wachusett Mountain on a well monitored bus by staff for the child's safety.

CONTRACT
The signed parent/guardian and the participant agree to the set rules by Wachusett Mountain and the
Recreation Department and also no contraband will be allowed, no disruptive and destructive
behavior will be allowed. Participants must be prepared to have backpacks and bags searched by the
Watertown Recreation staff. The safety of all must be observed. The student must a wear helmet
while skiing or boarding. If the participant does not follow the rules set by Wachusett Mountain and
the Recreation Department, he/she maybe removed from this program without a refund. The
parent/guardian may be asked to retrieve the participant from Wachusett Mountain, Princeton, MA.
