PERMIT/RESOLUTION FOR PUBLIC UTILITY INSTALLATION
IN CITY RIGHT OF WAY
WHEREAS US WEST COMMUNICATIONS , a public
utility licensed to do business in the City of Medina, her

to the City for the installation of Buried Cable Job orer 2�6�3
located at Hamel Rd_ and Pinto Dr_ and HamPL Rd. and Hunter Dr.
within the City of Medina;
NOW, THEREFORE, BE IT RESOLVED that said application be'granted
in accordance with the plans and specifications submitted, subject to the
following conditions:
1. Street and Right of Way shall be restored to their
original condition upon completion of Installation
and Applicant shall be liable for settlement or other
damage caused by said installation for a period of
the year following completion.
2. Completion Date:
3. Performance Bond in the Amount of:
4. Depth of Conduit.
5. No open cuts of road surface are allowed.
