PERMIT/RESOLUTION FOR PUBLIC UTILITY INSTALLATION
IN CITY RIGHT OF WAY
WHEREAS U.S. West Communications , a public
utility licensed to do business in the City of Medina, has made application
to the City- for the installation of buried telephone cable for plant improvement
located at Hamel Road between Tamarack and Arrowhead
within the City of Medina;
NOW, THEREFORE, BE IT RESOLVED that said application be.gr.anted
in accordance with the plans and specifications submitted, subject to
following conditions:
1. Street- and Right of Way shall be restored to their
original condition upon completion of Installation
and Applicant shall be liable for settlement or other
damage caused by said installation for a period of
the year following completion.
2. Completion Date:
3. Performance Bond in the Amount of:
d. Depth of Conduit.
5. No open cuts off any roads.

place buried cable on Hamel Road for plant improvement as shown
in detail on the attached sketches. This work will be done on
our Estimate number 22M5019.

approved copies of the sketches to this office.
