NOW, THEREFORE, BE IT RESOLVED by the Mayor and the Council of the Borough ofCarteret, County of Middlesex, State of New Jersey, as follows: 1, The proposal from CME Associates for Contract Phase Services related to the Carteret Health and Wellness Center, at a total cost not to exceed $10,000.00 is hereby approved, 2, This a{:tion is approved as an award of a professional services contract pursuant to N.J.S.A. 40A:ll~5(1)(a) in accordance with the Borough's Fair and Open Process as authorized by law. 3, A copy of this Resolution be forwarded to the Borough's Purchasing Department and CME Associates. As you are aware, our office prepared site plans to be utilized for construction of the Health and Wellness Center to be constructed by Matzel Development Company. Ills our understanding that the demolition work has commenced and the site work has been put out to bid ani;l that there is a pre.bli;li;lers conference tentatively scheduled for May 6, 2009 to review the site work with potential bidders, We feel that construction oversight of the site work should be provided by our office to ensure compliance with the approved construction plans, With this In mind we offer the following scope of services: Contract Phase Services Our office will provide site observation of the site improvements for the anticipated contract time including the following services: 1, Attend pre-bidders conference and pre-construction meeting; 2. Excentlons . Any environmental permitting beyond the scope outlined above will be considered extra. Said fees will be based on the hourly rate or a fixed fee mutually agreed upon writing, . The above-quoted fees will be In effect for a period of twelve (12) months from the date of this proposal; however, the fees on the hourly rate schedule are subject to change by January 1 of each year, Should you accept our fee estimate, kindly provide a purchase order or professional services resolution authorizing same.
