CITY OF MEDINA
RESOLUTION NO. qa-/oo
PERMIT/RESOLUTION FOR PUBLIC UTILITY INSTALLATION
IN CITY RIGHT OF WAY
WHEREAS U.S. West , a public utility
licensed to do business in the City of Medina, has made application
to the City for the installation of buried cable
located at Hamel Road, Tamarack Drive east to Pinto Drive
within the City of Medina;
NOW, THEREFORE, BE IT RESOLVED that said application be granted
in accordance with the plans and specifications submitted, subject to
the following conditions:
1. Street and Right of Way shall be restored to their
original condition upon completion of Installation
and Applicant shall be liable for settlement or other
damage caused by said installation for a period of
the year following completion.

The motion for the adoption in the foregoing Resolution is duly
seconded by Member Z i-el/oiv and upon vote being taken
