CALL TO ORDER: Dr. John Straus called the meeting to order at 7:35 PM.
PRESENT: Dr. John Straus, Dr. Barbara Beck, Steven Ward, and Kristel Bennett.
Minute Review: The Board of Health reviewed the minutes of the April 21, 2011.
Dr. Barbara Beck made a motion to accept the minutes of the April 21, 2011 meeting.
Dr. Straus stepped down as Chair and seconded the motion. All were in favor.
Always and Forever Tattoo: Kristel Bennett introduced Ms. Holly Azzara the owner of
Always and Forever Tattoo Studio at 565 Mt Auburn Street to the Board. Mrs. Bennett
presented the plan review documentation for Always and Forever Tattoo Studio and
informed the Board that Mr. Azzara is requesting a Tattoo Body Art Establishment and
Tattoo Practitioner Permit. Mrs. Bennett informed the Board that Ms. Azzara is currently
operating under a Temporary Tattoo Body Art Establishment and Temporary Tattoo
Practitioner Permit. Mrs. Bennett stated that the applicant has completed the plan review
process and has been instructed as to her responsibility concerning the inspection process.
Always and Forever Tattoo Studio has four workstations, two enclosed bathrooms, a
separate client waiting area, separate cleaning area, and separate storage area. Records
that are maintained by Ms. Azzara include Disclosure Statement, Emergency Plan,
Exposure Incident Report, Client Consent Forms, Exposure Control Plan, and Insurance
Liability Coverage. Ms. Azzara has completed has completed the following: Quincy's
Skin Course, First Aid and CPR certification, Blood Born Pathogen Training. Ms.
Azzara has been working as a permitted practitioner in other jurisdictions since 2002.
Ms. Azzara portfolio was submitted and had been reviewed.
After much discussion thereof Dr. Barbara Beck made a motion to approve the issuance
of a 2011 Tattoo Body Art Establishment Permit for Always and Forever Tattoo Studio

located at 565 Mt Auburn Street. In addition Dr. Barbara Beck made a motion to approve
the issuance of a 2011 Tattoo Practitioner Permit for Holly Azzara of Always and
Forever Tattoo Studio located at 565 Mt Auburn Street. Dr. John Straus stepped down as
Chair and seconded the motion. All were in favor.
67 Elton Avenue, Unit B: Kristel Bennett informed the Board of Health that the
owner of 67 Elton Avenue, Unit B, Mr. Richard Merullo has requested an administrative
hearing before the Board of Health for more compliance time with the order to correct
letter dated April 14, 2011. Mrs. Bennett presented the findings of the Health
Department inspection of the dwelling unit. Mr. Richard Merullo informed the Board
that the violations listed in the order to correct letter have been addresses. It came to light
that the dwelling unit was not approved use as an assessory apartment. After much
discussion thereof Dr. Barbara Beck made a motion to table further discussion of
compliance time until the owner goes before the zoning board of appeals for approval of
the dwelling unit as an assessory unit. Dr. Straus stepped down as Chair and seconded
the motion. All were in favor.
47 Hersom Street: Kristel Bennett informed the Board of Health that the owner of 47
Hersom Street, Mr. Wayne Sjostedt had requested an administrative hearing before the
Board of Health for more compliance time with the order to correct letter dated April 7,
2011. Mrs. Bennett presented the findings of the Health Department inspection of the
dwelling unit. Mr. Wayne Sjostedj informed the Board that the immediate violations
listed in the order to correct letter have been addressed. After much discussion thereof
Dr. Barbara Beck made a motion to amend the order to correct letter dated April 7, 2011
to extend the requirements of compliance time to June 1, 2011. If the violations in the
order to correct letter remain unabated by June 1, 2011 a court complaint will be filed
against the owner of 47 Hersom Street in Waltham District Court. Dr. John Straus
stepped down as Chair to second the motion. All were in favor.
175 Sycamore Street: Steven Ward informed the Board of Health that the owner of 175
Sycamore Street, Ellene Nelson had requested an administrative hearing before the Board
of Health for more compliance time with the order to correct letter dated April 12, 2011.

Mr. Steven Ward presented the findings of the Health Department inspection of the
dwelling unit. Ms. Ellene Nelson informed the Board that the violations listed in the
order to correct letter have been addressed. After much discussion thereof Dr. Barbara
Beck made a motion to amend the order to correct letter dated April 12, 2011 to extend
the requirements of compliance time to June 1, 2011. If the violations in the order to
correct letter remain unabated by June 1, 2011 a court complaint will be filed against the
owner of 175 Sycamore Street in Waltham District Court. Dr. John Straus stepped down
as Chair to second the motion. All were in favor.
22 Bates Road: Kristel Bennett informed the Board of Health that the owner of 22
Bates Road, Alice Nakashian had requested an administrative hearing for more
compliance time with the order to correct letter dated December 10, 2010. Mrs. Bennett
presented the findings of sanitarian, Mark Waksmonski's inspection of the dwelling and
presented Ms. Nakashian's submitted letter to address the remaining violations in the
dwelling. After much discussion thereof, Dr. Beck made a motion to amend the order to
correct letter dated December 10, 2010 to extend the requirements of compliance time to
the July 20, 2011 Board of Health meeting. At this meeting the owner will be required to
provide a status update to the Board of Health. Dr. John Straus stepped down as Chair to
second the motion. All were in favor.
18 Brimmer Street: Kristel Bennett presented the Case Management Agreement of 18
Brimmer Street to the Board. The Case Management Agreement has been developed, to
comply with the violations listed in the order to correct letter dated March 17, 2011 and
bring the dwelling unit into compliance by the July 20, 2011 Board of Health meeting.
The Board reviewed the Case Management Agreement.
Body Art: Steven Ward presented the proposed language for the Town of Watertown
Board of Health Body Art Regulations, Section 12(2)(a) and (b) as presented at the April
21, 2011 Board of Health meeting. After much discussion the Dr. Barbara Beck made a
motion to accept the proposed amendment as presented at the April 21, 2011 Board of
Health meeting without any changes. Dr. John Straus stepped down as Chair and
seconded the motion. All were in favor.

403 School Street: Kristel Bennett informed the Board of Health that condemnation
order of February 23, 2011 had been withdrawn since the May 5, 2011 inspection
revealed the conditions noted on February 23, 2011 no longer existed. The May 5, 2011
inspection revealed additional violations such as no heat, no smoke or carbon monoxide
detectors, no water supply at kitchen faucet, bathroom faucet, or tub; as a result an order
letter was issued for the violations in the dwelling, and the dwelling was rendered unfit
for human habitation.
