A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF .
LYNWOOD APPROVING OF AND MAKiNG APPLICATION FOR
THE. DISABILITY RETIREMENT OF~LYNWOOD CITY EMPLOYEE
~. DONALD R. BROWN PURSUANT: TO SECTIONS 2102 AND
. 21025 OF THE GOVERNMENT CODE.
~: WHEREAS, the City of Lynwood (hereinafter referred to as `
"Agency'!)- is a contracting agency of the Public Employees;' .
Retirement Sy"stem; and.
"' WHEREAS, ,the Public Employees' Retirement Law requires
- that a contracting agency determine whether an employee of such -.
agency in employment in which he is classified as~a local safety
~~ member is-disabled for purposes of the Public Employes' .-~
~.'Retiremert Law and whether such disability is "Industrial" within
t-he meaning of such haw;. and ~ •
WHEREAS, the:City of Lynwood has reviewed. ,the medical and ..
other edidence'-relevant to .such alleged disability.
NOW,~THEREFORE, BE IT RESOLVED, that-the City of Lynwood
find and determine, and' it does hereby find 'and determine that.. .
. DONALD .R. BROWN is incapacitated within the meaning ;of the
<~Public`Employees' Retirement Law. for performance of his duties
in the position of Police Officer and establish August 8,.1977
as his .effective date of retirement; and
BE IT-FURTHER RESOLVED that the-.City of Lynwood Y~ereby
` `makes application for the disability retirement of .DONALD R. BROWN ;
and authorizes and. directs its appropriate officers to execute
all documents necessary and convenient therefor; and
• .. ~ BE°IT FURTHER RESOLVED that the City of Lynwood find and
determine and it ,does hereby find and determine that such
disability is~a'result of injury or disease arising out of and.
in the course of.employrnent. `
