BMP 2023 ANNUAL MAINTENANCE REPORT FORM
The Post -Construction Stormwater Best Management
Practice (BMP) Annual Maintenance Report is required for
demonstrating compliance with the Maintenance
Agreement and Stormwater Management Facilities Report
for your property development. MSD provides this online
report form to Stormwater BMP owners as a consistent and
convenient option to meet their annual reporting
obligations. To comply with the annual report requirement,
please submit one report form for each MSD
Project/Record Number at your site. Please answer all
questions for each stormwater BMP facility at your property
development. Instructions for completing certain fields will
display by hovering over the ? symbol with the pointer.

trash, debris or other potential pollutants exposed to stormwater.

Describe the condition of the inlet(s), or area conveying sheet flow if piped inlets do not exist.*
Good condition. Inlet(s) effectively conveys water into the facility and there is no evidence of erosion, sediment
accumulation, trash and other debris or pollutants.
Moderate condition. Inlet(s) conveys water into the facility, but there is some erosion, sediment accumulation, trash
and other debris or pollutants.
Degraded condition. Inlet(s) conveys water into the facility, but there are areas degraded by erosion, sediment
accumulation, trash and other debris or pollutants.
Serious condition. Inlet(s) may or may not convey water into the facility, and there are significant and problematic
areas of erosion, sediment accumulation, trash and other debris or pollutants.
