BMP 2023 ANNUAL MAINTENANCE REPORT FORM
The Post -Construction Stormwater Best Management
Practice (BMP) Annual Maintenance Report is required for
demonstrating compliance with the Maintenance
Agreement and Stormwater Management Facilities Report
for your property development. MSD provides this online
report form to Stormwater BMP owners as a consistent and
convenient option to meet their annual reporting
obligations. To comply with the annual report requirement,
please submit one report form for each MSD
Project/Record Number at your site. Please answer all
questions for each stormwater BMP facility at your property
development. Instructions for completing certain fields will
display by hovering over the ? symbol with the pointer.

Describe the condition of the contributing drainage area.*
Good condition. Contributing drainage is free of erosion, sediment or gravel, trash, debris and other potential
pollutants.
Moderate condition. Contributing drainage has some areas with erosion, sediment or gravel, trash, debris and other
potential pollutants.
Degraded condition. Contributing drainage contains areas degraded by erosion, sediment or gravel, trash, debris and
other potential pollutants.
Serious condition. Contributing drainage contains significant and problematic areas of erosion, sediment or gravel,
trash, debris or other potential pollutants exposed to stormwater.
Not Applicable
Describe the condition of the inlet(s), or area conveying sheet flow if piped inlets do not exist.*
Good condition. Inlet(s) effectively conveys water into the facility and there is no evidence of erosion, sediment
accumulation, trash and other debris or pollutants.
Moderate condition. Inlet(s) conveys water into the facility, but there is some erosion, sediment accumulation, trash
and other debris or pollutants.
Degraded condition. Inlet(s) conveys water into the facility, but there are areas degraded by erosion, sediment
accumulation, trash and other debris or pollutants.
Serious condition. Inlet(s) may or may not convey water into the facility, and there are significant and problematic
areas of erosion, sediment accumulation, trash and other debris or pollutants.
Not Applicable
Describe the condition of the pretreatment area(s) or forebay(s). *
Good condition. There is no evidence of sediment/trash/debris, erosion, standing water or clogging, dead vegetation
or bare soil. Adequate condition of structural components.
Moderate condition. There is some sediment/trash/debris, erosion, standing water or clogging, dead vegetation or
bare soil. Adequate condition of structural components.
Degraded condition. There are areas degraded by sediment/trash/debris, erosion, standing water or clogging, dead
vegetation or bare soil. Condition of structural components may be inadequate.
Serious condition. There are significant and problematic areas of sediment/trash/debris, erosion, standing water or
clogging, dead vegetation or bare soil. Condition of structural components may be inadequate.
Not Applicable
Describe the condition of the treatment area, and its vegetation if applicable.*
Good condition. There is no evidence of sediment/trash/debris, erosion, missing mulch or pea gravel, oil/chemical
accumulation, standing water or clogging, dead vegetation or bare soil. There is adequate plant covering, vegetation is
not overgrown, there is no evidence of invasive, overgrown weeds, or woody vegetation. Adequate condition of
structural components.
Moderate condition. There is some sediment/trash/debris, erosion, missing mulch or pea gravel, oil/chemical
accumulation, standing water or clogging, dead vegetation or bare soil. There is some inadequate plant covering,
overgrown vegetation, and invasive, overgrown weeds, or woody vegetation. Adequate condition of structural
components.
Degraded condition. There are degraded areas of sediment/trash/debris, erosion, missing mulch or pea gravel,
oil/chemical accumulation, standing water or clogging, dead vegetation or bare soil. There are degraded areas of
inadequate plant covering, overgrown vegetation, and invasive, overgrown weeds, or woody vegetation. Condition of
structural components may be inadequate.
Serious condition. There are significant and problematic areas of sediment/trash/debris, erosion, missing mulch or
pea gravel, oil/chemical accumulation, standing water or clogging, dead vegetation or bare soil. There are significant and
problematic areas of inadequate plant covering, overgrown vegetation, or invasive, overgrown weeds, or woody
vegetation. Condition of structural components may be inadequate.
Not Applicable
Describe the condition of the overflowloutlet structure. *
Good condition. Overflow/outlet effectively conveys water out of the facility and there is no evidence of erosion,
sediment/trash/debris or other pollutants. Adequate condition of structural components.
Moderate condition. Overflow/outlet conveys water out of the facility and there is some evidence of erosion,
sediment/trash/debris or other pollutants. Adequate condition of structural components.
Degraded condition. Overflow/outlet conveys water out of the facility, but there are degraded areas of erosion,
sediment accumulation, trash and other debris or pollutants. Condition of structural components may be inadequate.
Serious condition. Overflow/outlet may or may not convey water out of the facility, and there are significant and
problematic areas of erosion, sediment accumulation, trash and other debris or pollutants. Condition of structural
components may be inadequate.
