BMP 2023 ANNUAL MAINTENANCE REPORT FORM
The Post -Construction Stormwater Best Management
Practice (BMP) Annual Maintenance Report is required for
demonstrating compliance with the Maintenance
Agreement and Stormwater Management Facilities Report
for your property development. MSD provides this online
report form to Stormwater BMP owners as a consistent and
convenient option to meet their annual reporting
obligations. To comply with the annual report requirement,
please submit one report form for each MSD

questions for each stormwater BMP facility at your property

Construction for this project was approved on July 18, 2016 and includes two vegetative filter strips used to stabilize the slopes of Access Road 1C at the
box culvert replacement. The vegetative strips are underlain by coconut erosion control blanket and Flexamat and planted with native grass seed.
Inspections have revealed that the edges of both slopes, as well as gaps between Flexamat rolls, are susceptible to erosion from the large volumes of
stormwater that drain down the slope across the Flexamat and at the edges. Multiple areas of erosion have been addressed several times since
inspections first began in September 2016, but erosion on the north slope continues to go unchecked. After carefully reviewing each component of the
system and finding that routine maintenance items are needed (listed below), I can confirm:

March 2, 2022: Slight erosion / was noted in the rip rap covered area at the east end of the south slope that should be monitored.
June 15, 2022: Vegetation has regrown. Native seeded grasses has good coverage across both slopes. The east half of the south slope has become
overgrown. In its current state, further erosion and scouring of the rip rap adjacent to the Flexamat could not be assessed with the vegetation overgrown
with vines, poison ivy, and saplings. This should be cut down prior to the September inspection so a full evaluation can be made.
September 21, 2022: Erosion on the north slope continues. Erosion at the lower west slope appears to be marching up the middle seam and getting
worse as time passes. Repair and intervention is needed. Good coverage of seeded grasses remains on parts of both slopes. Bush clover is taking over
especially on the south slope, but also the middle top of the north slope. Although invasive, it is considered useful for erosion control so can remain in
place. One drawback, however, is that it grows so thickly it really prevent viewing the Flexamat and assessing for erosion or settlement. Sycamore and
honeysuckle saplings are growing on both slopes. These should be managed. If allowed to grow the maturity, root growth could displace the Flexamat
and cause erosion.
December 9, 2022: Sapling growth was cut down and removed since the previous inspection. With vegetation now dormant, visibility had increased at
the overgrown rip rap area on the east side of the south slope. There was an area about 2 feet wide that is scouring out and could use more rip rap.
Walked the site with James and Jason to point out the areas of erosion.
March 8, 2023: No changes noted.
June 8, 2023: Excessive sediment, gravel and rock that had collected at the edges of the roadway was removed.
September 21, 2023: Erosion continues on the south slope along the middle seam and at the lower west edge. Condition of erosion at the east edge of
the south Flexamat was not observed as the rip rap is highly overgrown with saplings, vines and weeds. Sapling growth should be cut down and removed,
and the stems treated with herbicide. The concern is that as trees grow up through the Flexamat it will distort the erosion control blanket.
December 7, 2023: Sapling growth on both Flexamat slopes was cut down and removed.
