A RESOLUTION OF THE CITY COMMISSION OF THE
CITY OF OPA-LOCKA, FLORIDA, AUTHORIZING THE
CITY MANAGER TO ACCEPT A TECHNICAL
ASSISTANCE FUNDING GRANT FROM THE ALLIANCE
FOR A HEALTHIER GENERATION IN THE AMOUNT
OF $1,500 TO $3,000, AND A MEMORANDUM OF
UNDERSTANDING ("MOU") TO SERVE AS A HOST SITE
FOR CHILD AND YOUTH DEVELOPMENT PROGRAMS
AT SHERBONDY PARK FOR HEALTH AND WELLNESS
EDUCATION; PROVIDING FOR INCORPORATION OF
RECITALS; AND PROVIDING FOR AN EFFECTIVE
DATE
WHEREAS, Alliance for a Healthier Generation ("Alliance") is a program providing
out-of-school time sites with access to a variety of experts, resources and support to help
achieve the Healthy Out-of-school Time ("HOST"), Framework Standards and Best Practices,
to improve healthy eating and physical activity, to reduce the prevalence of childhood obesity
and empower kids to develop lifelong, healthy habits; and
WHEREAS,the City Commission of the City of Opa-locka desires the City of Opa-
locka Child and Youth Development Program at Sherbondy Park to accept and participate as a
host site for the Alliance for a Healthier Generation Healthy Out-of-School Time Program; and
WHEREAS, the City Commission of the City of Opa-locka desires to authorize the City
Manager to accept a Technical Assistant Funding Grant from the Alliance for a Healthier
Generation in the amount $1,500 to $3,000, and enter into a Memorandum of Understanding
("MOU") with Alliance; and
WHEREAS, the funds will be used to promote healthy eating habits and wellness
education for youths between the ages of 8 to 18 years of age.

NOW, THEREFORE, BE IT DULY RESOLVED BY THE CITY COMMISSION
OF THE CITY OF OPA LOCKA, FLORIDA:
Section 1. The recitals to the preamble are hereby incorporated by reference.
Section 2. The City Commission of the City of Opa-locka, authorizes the City
Manager to accept a Technical Assistance Funding Grant from the Alliance for a Healthier
Generation in the amount of$1,500 to $3,000, and enter into a MOU with Alliance to serve as a
host site for child and youth development programs at Sherbondy Park for health and wellness
education.
Section 3. This resolution shall take effect immediately upon adoption.

Request: A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF OPA-
LOCKA AUTHORIZING THE CITY MANAGER TO ACCEPT A TECHNICAL
ASSISTANCE FUNDING GRANT FROM THE ALLIANCE FOR A HEALTHIER
GENERATION IN THE AMOUNT OF $1,500 TO $3,000, (ONE THOUSAND
FIVE HUNDRED DOLLARS TO THREE THOUSAND DOLLARS) AND
MEMORANDUM OF UNDERSTANDING TO SERVE AS A HOST SITE FOR
CHILD AND YOUTH DEVELOPMENT PROGRAMS AT SHERBONDY PARK
FOR HEALTH AND WELLNESS EDUCATION.
Description: The proceeds from this award will be used to promote healthy eating habits and wellness
education for all children and youth between the ages of eight (8) to eighteen (18) years
of age in order that young enough they will know the value of good choices for eating,
while promoting healthier changes for a lifetime.
Financial Impact: No Fiscal Impact. One hundred percent of the proceeds are grant dollars.

Framework standards and best practices. Alliance for a Healthier Generation staff is committed to
facilitating your organization's efforts to develop local solutions and strategies for creating change.
The Alliance for a Healthier Generation agrees to:
• Provide support and training to out-of-school time sites to support their efforts to improve healthy
eating and physical activity
• Broker local, state and national resources to support out-of-school time sites in implementing an
Action Plan
• Celebrate the achievements of out-of-school time sites in meeting the Healthy Out-of-School Time

• Communicate regularly with the Alliance for a Healthier Generation HOST Manager
• Participate in Alliance for a Healthier Generation evaluation activities, as requested
• Commit to participating with the Alliance for a Healthier Generation until August 30, 2014
Nothing in this memorandum of understanding shall be deemed to be a commitment or obligation of
Alliance for a Healthier Generation or out-of-school time site funds. The Alliance for a Healthier
Generation reserves the right to terminate this agreement in its sole discretion if it does not receive
continued sufficient external funding.

Alliance HOST Manager Signature:
*The Alliance strongly encourages each out-of-school time site to secure at least two signatures on the
MOU and for these individuals to participate throughout the Six Step Process. Building support at the site-
level and organizational or leadership-level for the Healthy Out-of-School Time Initiative will help to maintain
momentum during the Six Step Process and help to ensure long-term sustainability of the healthy changes
made. It is recommended that the Primary Site Contact be a permanent employee of the OST site, rather
than a volunteer or temporary employee. It is recommended that the Organization or Leadership-level
contact be a representative that can support the work of the site and help ensure long term sustainability of
changes, such as the OST Site Director, Regional Program Director or Executive Director/CEO.
