A RESOLUTION OF THE CITY COMMISSION OF THE CITY
OF OPA-LOCKA,FLORIDA,TO AMEND RESOLUTION NO
05-6744 EMPLOYEE PERFORMANCE EVALUATION FORM,
UTILIZED TO EVALUATE THE EMPLOYEES IN THE
POSITIONS OF CITY MANAGER, CITY ATTORNEY AND
CITY CLERK, ON OCTOBER 1ST OF EACH CALENDAR
YEAR; PROVIDING INCORPORATION FOR RECITALS;
PROVIDING AN EFFECTIVE DATE
WHEREAS,the City Commission of the City of Opa-locka desire to amend Resolution No.
05-6744, the Evaluation Form used to evaluate the City Manager, City Clerk and City Attorney.
NOW,THEREFORE,BE IT DULY RESOLVED BY THE CITY COMMISSION OF
THE CITY OF OPA-LOCKA, FLORIDA:
Section 1. The recitals to the preamble herein are incorporated by reference.
Section 2. The City Commission of the City of Opa-locka,Florida,approves amending
Resolution 05-6744,to amend the Evaluation Form used to evaluate the City's three(3) appointed
employees: the City Manager, City Clerk, and City Attorney, in the form attached hereto.
Section 3. This Resolution shall take effect immediately upon adoption.

The City of Opa-locka is encouraged to establish a system of performance evaluation for each
appointed official and will be rated by the City Commission. This appraisal may afford the
appointed official an opportunity to clarify their job performance accomplishments. If no
improvement in job performance is noted with the City of Opa-locka; this individual may be
considered for termination. The objective is to attain documented achievements that directly
impact measurable outcomes, which ultimately enhances the City's overall effectiveness
through branding, revenue generation, business enhancement, resident concerns, employee's
leverage to the overall City's accomplishments, etc.

INSTRUCTIONS
This evaluation form contains ten categories of evaluation criteria. Each category contains a
statement to describe a behavior standard in that category. For each statement, use the
following scale to indicate your rating of the Appointed Official performance.
5 = Excellent (almost always exceeds the performance standard)
4=Above average (generally exceeds the performance standard)
3 =Average (generally meets the performance standard)
2 = Below average (usually does not meet the performance standard)
1 = Poor (rarely meets the performance standard)
Any item left blank will be interpreted as a score of"3 =Average"
This evaluation form also contains a provision for entering narrative comments, including an
opportunity to enter responses to specific questions and an opportunity to list any comments
you believe appropriate and pertinent to the rating period. Please write legibly.

OPA-LOCKA, FLORIDA APPROVING AND ADOPTING THE
EMPLOYEE PERFORMANCE EVALUATION FORM
ATTACHED HERETO TO BE UTILIZED IN EVALUATING THE
EMPLOYEES IN THE POSITIONS OF CITY MANAGER, CITY
CLERK AND CITY ATTORNEY ON JULY 1 OF EACH
CALENDAR YEAR
WHEREAS, the positions of the city manager,city clerk,and city attorney report to and are
under the supervision of the City Commission; and
WHEREAS, the City Commission wishes to provide an employee performance evaluation
form for purposes of evaluating those employees serving in the positions of city manager,city clerk
and city attorney
NOW,THEREFORE,BE IT DULY RESOLVED BY THE CITY COMMISSION OF THE
CITY OF OPA-LOCKA, FLORIDA:
Section 1. The recitals to the preamble herein are incorporated by reference.
Section 2. The City Commission hereby approves and adopts the employee performance
evaluation form attached hereto to be utilized in evaluating the employees in the positions of city
manager, city clerk and city attorney on July 1 of each calendar year.
