ADDENDUM NO. 2
THIS ADDENDUM, (Number 2 to Contract No. 124-2008) is made and entered into this __18th __ day of _November________, 2010, by and between Richmond, Indiana, a municipal corporation
acting through its Board of Public Works and Safety with its office at 50 North 5th Street, Richmond, Indiana, (hereinafter referred to as “City”), and Corporate Health of Ohio, Inc.,
d/b/a Public Safety, Health & Wellness, 235 Industrial Drive, Franklin, Ohio, 45005 (hereinafter referred to as “Contractor”).
WHEREAS, City and Contractor entered into Contract No. 124-2008, dated November 6, 2008, which Contract is a one-year contract containing two annual options to renew for a second
calendar year in 2010 and a third calendar year in 2011; and
WHEREAS, City wishes to retain Contractor to continue providing physical examinations to the City of Richmond Firefighters, including but not limited to general physical examinations,
pulmonary function tests, vision and hearing tests, and other standard medical testing during the 2011 calendar year within the scope of Contract No. 124-2008.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained, including the above stated recitals, City and Contractor hereby agree as follows:
Contractor’s address for purposes of correspondence shall be 235 Industrial Drive, Franklin, Ohio, 45005.
The City and Party executed and entered into Contract No. 124-2008, on November 6, 2008. Contract No. 124-2008 is attached hereto, marked as Exhibit “A” and incorporated by reference.
Pursuant to Contract No. 124-2008, Section IV Term of Agreement, the City has an option to renew the Agreement for a third year (the 2011 calendar year), which option is exercised pursuant
to this Addendum. Contractor indicated its willingness to extend Contract No. 124-2008 for the 2011 calendar year on October 12, 2010, which agreement is attached hereto, marked as
Exhibit “B” and incorporated by reference.
Both parties hereby agree to renew Contract No. 124-2008 with the same terms and conditions set forth in that Agreement, with the exception that the date of the term shall be extended
to December 31, 2011.
Both parties hereby agree that with the exception of the extended dates, Contract No. 124-2008 shall remain intact and unchanged.
IN WITNESS WHEREOF, this Addendum Number 2 to Contract No. 124-2008 is executed and approved by the undersigned representatives of the City and Contractor this _18th ___, day of _November______,
2010 to be effective in accordance with its terms as of January 1, 2011.
