A RESOLUTION OF THE MAYOR AND CITY
COMMISSION OF THE CITY OF OPA LOCKA, FLORIDA,
RATIFYING AND APPROVING AN AGREEMENT
BETWEEN THE CITY OF OPA-LOCKA AND EAC
CONSULTING, INC., FOR ADDITIONAL SCOPES OF
SERVICES, IN AN AMOUNT NOT TO EXCEED FORTY-
FIVE THOUSAND DOLLARS ($45,000.00), PAYABLE FROM
ACCOUNT NUMBER 75-5356314; AND AUTHORIZING
THE CITY MANAGER TO EXECUTE ALL DOCUMENTS
NECESSARY TO EFFECTUATE SAID AGREEMENT;
PROVIDING FOR INCORPORATION OF RECITALS;
PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, The City of Opa-locka, Florida is in need of engineering services
for evaluation and compliance with the Clean Water State Revolving Fund (SFR) ;
WHEREAS, EAC Consulting has previously performed phases of the services
required and can effectively continue to provide the services as needed; and
WHEREAS, It has been determined that it is in the best interest of the City to
allow EAC Consulting to continue to provide services to the City.
NOW THEREFORE, BE IT RESOLVED THAT THE CITY COMMISSION
OF THE CITY OF OPA LOCKA, FLORIDA:
Section 1. The recitals to the preamble herein are incorporated by reference;
Section 2. The City Commission approves and ratifies the agreement between
the City of Opa-Locka and EAC Consulting that would require EAC Consulting to
conduct Sanitary Sewer Evaluation Survey Phase I and II, Flow and Smoke Testing,
and Manhole Inspection of approximately 56,000 linear feet to comply with the
Clean Water State Revolving Fund (SRF) planning requirements in an amount not to
exceed Forty-five Thousand Dollars ($45,000.00), payable from account number 75-
5356314.

Section 3. The City Manager is hereby authorized to execute all documents
necessary to effectuate agreement.
Section 4. This resolution shall take effect immediately upon adoption

The purpose of this Request for approval and ratification of additional Sanitary Sewer Evaluation Survey

APROXIMATELY 56,000 LINEAR FEET TO COMPLY WITH THE CLEAN

THE AMOUNT NOT TO EXCEED FORTY FIVE THOUSAND FIVE HUNDRED
DOLLARS ($45,500.00), PAYABLE FROM ACCOUNT NUMBER 75-5356314

Description: The purpose of the change in scope was to perform the flow testing, smoke testing,
manhole inspection, etc. to approximately 56,000 linear feet which was not included in the original scope
for the wastewater infrastructure system project that EAC Consulting, Inc. provided to the City under the
contract agreement dated October 24, 2013. EAC Consulting was awarded the contract as consultant for
this project under RFQ No. 13-1104100, performed this task in accordance to the Clean Water State
Revolving Fund (SRF) Planning Requirements. This helped the city meet the DERM requirement in a
timely manner. All sanitary sewer collection and transmission system are required to complete a Sanitary
Sewer Evaluation Survey (SSES) according to the guidelines published in the U.S. EPA's sewer system
Infrastructure Analysis and Rehabilitation Handbook (1991), EPA/625/6-91/030. As part of the
agreement EAC Consulting, Inc. Prepared Phase I which consisted of the evaluation of the wastewater
collection system to determine the amount of existing infiltration and inflow(I/I), and the Phase II which
is the plan for upgrading the collection system to meet or exceed the minimum I/I standards. The
completion of an SSES is required followed by sewer rehabilitation and post-rehabilitation flow
monitoring to determine compliance with 5,000 gallons per day-inch-mile (GPDIM) standard for(UI) as
specified in the Miami Dade County Code.
Account Number: 75-5356314.
Financial Impact: This additional scope is unbudgeted. However,the fee of$45,500.00 was approved
and paid by the Department of Environmental Protection, State Revolving Fund loan.
Implementation Time Line: As soon as possible since we need to provide DERM with our SSES
Report.
Legislative History: RFQ No. 13-1104100 awarded July 30th 2013.
Recommendation(s): Staff recommends approval in order to facilitate the requirements of DERM. As
this is part of the requirement for the consent decree as it relates to the Sanitary Sewer Evaluation Survey
(SSES).
Analysis:
Based on our Water and Sewer Atlas the additional 56,000 linear feet were not included. To have a
complete picture of our Sanitary Sewer this needed to be done.

INTO AN AGREEMENT WITH EAC CONSULTING,
INC., FOR THE PREPARATION OF WASTEWATER
MASTER PLAN, IN AN AMOUNT NOT TO EXCEED
N512,806.25, PAYABLE FROM ACCOUNT NUMBER
35-535340; PROVIDING FOR INCORPORATION OF
RECITALS; PROVIDING FOR AN EFFECTIVE DATE
WHEREAS, the City of Opa-locka ("City") requested proposals for the Capital
Improvement Project("CIP") Wastewater Master Plan; and
WHEREAS, the City, by Resolution 13-8595, authorized the City Manager to
negotiate a contract for the Wastewater Master Plan with EAC Consulting Inc. ("EAC");
and
WHEREAS, the Wastewater Master Plan task is a 10 year Capital Improvements
Program, targeting wastewater facilities infrastructure, and will recommend solutions, and
address present deficiencies and future demands; and
WHEREAS, EAC Consulting, Inc.'s, services for the Wastewater Master Plan
will prioritize and evaluate the costs associated with the necessary improvements, and will
provide guidance with internal and external funding opportunities; and
WHEREAS, EAC will develop an individual wastewater infrastructure report, and
solutions will be proposed for implementation within a 10 year period to correct
deficiencies and propose solutions based on future demands; and

WHEREAS,the City Manager recommends City Commission approval, in order
to move ahead with the Capital Improvement Program.
NOW, THEREFORE, BE IT DULY RESOLVED BY THE CITY
COMMISSION OF THE CITY OF OPA-LOCKA, FLORIDA:
Section 1. The recitals to the preamble herein are incorporated by reference.
Sect ion 2. The City Commission of the City of Opa-locka, hereby approves
and authorizes the City Manager to enter into an agreement with EAC Consulting,Inc., for
the preparation of the Wastewater Master Plan, in an amount not to exceed $512,806.25,
payable from Account Number 35-535340, in substantially the form attached hereto.
Section 3. This resolution shall take effect immediately upon adoption.

Dear Mr. Nasir,
EAC Consulting Inc. is pleased to submit the following scope and fees for this amendment to provide
Engineering Consulting Services for the development of the City of Opa-Locka's Sanitary Sewer Evaluation
Survey Phases 1 and 2 reports for Lift Stations 3D, 11A, and 11D.
Our services on this project will be supported by our subconsultant—
• C&I Construction and Design, Inc.- Flow Testing,Smoke test, Manhole inspection,etc
Sanitary Sewer Evaluation Survey(Phase I and II)
This task will be performed according to the Clean Water State Revolving Fund (SRF) Planning
Requirements.All sanitary sewer collection and transmission systems are required to complete a Sanitary
Sewer Evaluation Survey (SSES) according to the guidelines published in the U.S EPA's Sewer System
Infrastructure Analysis and Rehabilitation Handbook(1991),EPA/625/6-91/030. The SSES shall be carried
out in three phases. Phase I is an evaluation of the wastewater collection and transmission system to
determine the amount of existing Infiltration and Inflow. (I/I). Phase II is the plan for upgrading the
collection system to meet or exceed the minimum I/I standards. Phase III is the implementation of the
plan developed in Phase II and includes remeasurement to prove that the collection system is operating
at an acceptable level.
The City desires to continue providing reliable wastewater collection services for new development and
to enhance their existing system and has requested that the EAC Team perform an SSES for the City's
collection system which includes an additional 56,000 linear feet of sanitary sewer and additional
manholes for these collection basins. The completion of an SSES is required followed by sewer
rehabilitation and post-rehabilitation flow monitoring to determine compliance with 5,000 gallons per
day-inch-mile (gpdim) standard for I/I as specified in the Miami Dade County Code. Phase I (SSES) must
include a system survey and flow measurement to identify any collection basins that exceed 5,000 gpdim.
Phase II of the SSES includes a Rehabilitation Plan which should identify specific repairs that will reduce
I/I.
Excluded from this proposal is Phase 3,which is work to be undertaken at a future time by the City. Phase
III includes conducting post-rehabilitation flow measurement to verify and demonstrate compliance with
the Miami Dade County Code.
Please find enclosed the following attachment to this fee proposal letter:

As part of this,the City is expected to provide access throughout the project to engineering and operations
staff for field visit accompaniment when required and interviews to help supplement existing data and
document existing conditions.Specific information relative to operations,such as time stamps, measured
volumes, run times,etc shall be provided to EAC as requested from the City.
The following activities will be performed under this task:
A. Flow Monitoring Data Collection for Basin Prioritization
B. Flow Monitoring Data Analysis for Basin Prioritization
C. Basin Prioritization Report. Note that this is also a Miami Dade County DRER requirement.
D. Smoke Testing. The EAC team will include a professional vendor for this task. Smoke testing will
involve testing the existing gravity sewer mains using a high volume blower along with smoke
canisters or liquid smoke to generate the smoke. Smoke will be non-toxic odorless and non-
staining.
E. Night Flow Isolation Analyses and Studies.
F. Manhole Inspections
G. Compilation of Inspection Findings and Reports (Phase 1 Report). Organize field data using a
common and user friendly database software system. Process, evaluate and electronically store
the results of manhole inspection and other inspection activities as applicable

Based upon Applications for Payment,submitted to the Owner/Agent by the Contractor,the Owner/Agent shall make progress payments on account of the Contract Sum to

D. Smoke Testing. The EAC team will include a professional vendor for this task.Smoke testing will
involve testing the existing gravity sewer mains using a high volume blower along with smoke
canisters or liquid smoke to generate the smoke. Smoke will be non-toxic odorless and non-
staining.
E. Night Flow Isolation Analyses and Studies.
F. Manhole Inspections
G. Compilation of Inspection Findings and Reports (Phase 1 Report). Organize field data using a
common and user friendly database software system. Process,evaluate and electronically store
the results of manhole inspection and other inspection activities as applicable

Dear Mr.Nasir,
EAC Consulting Inc. is pleased to submit the following scope and fees for this amendment to provide
Engineering Consulting Services for the development of the City of Opa-Locka's Sanitary Sewer Evaluation
Survey Phases 1 and 2 reports for Lift Stations 3D,11A,and 11D.
Our services on this project will be supported by our subconsultant—
• C&I Construction and Design, Inc.-Flow Testing,Smoke test,Manhole inspection,etc
Sanitary Sewer Evaluation Survey(Phase I and II)
This task will be performed according to the Clean Water State Revolving Fund (SRF) Planning
Requirements.All sanitary sewer collection and transmission systems are required to complete a Sanitary
Sewer Evaluation Survey (SSES) according to the guidelines published in the U.S EPA's Sewer System
Infrastructure Analysis and Rehabilitation Handbook(1991),EPA/625/6-91/030. The SSES shall be carried
out in three phases. Phase I is an evaluation of the wastewater collection and transmission system to
determine the amount of existing Infiltration and Inflow. (I/I). Phase II is the plan for upgrading the
collection system to meet or exceed the minimum I/I standards. Phase III is the implementation of the
plan developed in Phase II and includes remeasurement to prove that the collection system is operating
at an acceptable level.
The City desires to continue providing reliable wastewater collection services for new development and
to enhance their existing system and has requested that the EAC Team perform an SSES for the City's
collection system which includes an additional 56,000 linear feet of sanitary sewer and additional
manholes for these collection basins. The completion of an SSES is required followed by sewer
rehabilitation and post-rehabilitation flow monitoring to determine compliance with 5,000 gallons per
day-inch-mile(gpdim)standard for I/I as specified in the Miami Dade County Code. Phase I (SSES) must
include a system survey and flow measurement to identify any collection basins that exceed 5,000 gpdim.
Phase 11 of the SSES includes a Rehabilitation Plan which should identify specific repairs that will reduce
I/I.
Excluded from this proposal is Phase 3,which is work to be undertaken at a future time by the City. Phase
III includes conducting post-rehabilitation flow measurement to verify and demonstrate compliance with
the Miami Dade County Code.

As part of this,the City Is expected to provide access throughout the project to engineering and operations
staff for field visit accompaniment when required and interviews to help supplement existing data and
document existing conditions.Specific information relative to operations,such as time stamps,measured
