THIS ADDE UM, (Number 1 to Contract No. 38-2015) is made and entered into this
day of 2015, by and between Richmond, Indiana, a municipal
corporation acting throu'o its Board of Public Works and Safety with its office at 50 North 5`s Street,
Richmond, Indiana, 47374 (hereinafter referred to as "City"), and Advanced Rescue Solutions, LLC,
7098 Hickory Hollow Court, Plainfield, Indiana, 46168-8749 (hereinafter referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 38-2015 on April 23, 2015, which
Agreement retains Contractor to provide professional services in connection with the
Rope Rescue Course and Confined Space Rescue Course for the City of Richmond
Fire Department; and
WHEREAS, The parties to said Agreement wish to add additional spaces for participants to the
Agreement under the same terms and conditions as set forth in Contract No. 38-
2015.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
1. The City and Contractor executed and entered into Contract No. 38-2015, on April
23, 2015. Pursuant to Contract No. 38-2015, Section XI Miscellaneous, the
Agreement may be altered or amended in whole or in part at any time by filing with
the Agreement a written instrument setting forth such changes signed by both parties.
As set forth above, additional spaces shall be included for the courses so that nine (9)
additional participants shall be provided with the training as set forth within
• Contract No. 38-2015. Such services shall be performed in a manner consistent with
Contract No. 38-2015.
2. Section III Compensation of said Agreement shall be altered in order that City shall
pay Contractor an additional amount of Two Thousand Seven Hundred Dollars and
Zero Cents ($2,700.00) for the additional participants, with said final contract amount
to be increased to a total amount not to exceed Twenty-four Thousand Seven
Hundred Dollars and Zero Cents ($24,700.00).
City and Contractor expressly agree that all other terms, conditions, and covenants
contained in Contract No. 38-2015, shall be applicable to the work performed
pursuant to this Addendum, unless such term, condition or covenant conflicts with
this Addendum Number 1, in which case this Addendum Number 1 shall be
controlling.
4. Both parties hereby agree that with the exception of the addition of the above
described additional participants, Contract No. 38-2015 shall remain intact and
unchanged and in full force and effect.

5. Both parties agree that any person executing this Contract in a representative Capacity
hereby warrants that he or she has been duly authorized by his or her principal to
execute this Addendum.
