RESOLUTION NO. 16-9234
A RESOLUTION OF THE MAYOR AND CITY
COMMISSION OF THE CITY OF OPA-LOCKA, FLORIDA,
AUTHORIZING THE CITY MANAGER TO SPEND $1,400.00
FOR TWO SURVEYS NEEDED TO COMPLETE THE PUBLIC
UTILITIES INSTALLATION AND RESURFACING
PROJECT ON CAIRO LANE; PROVIDING FOR
INCORPORATION OF RECITALS; PROVIDING FOR AN
EFFECTIVE DATE.
WHEREAS, The City is in the process of installing public utilities and
resurfacing the road on Cairo Lane; and
WHEREAS, The City needs to conduct two surveys on Cairo Lane in order to
complete the required public utilities installation and resurfacing; and
WHEREAS,The cost of the two surveys is $1,400.00 (Exhibit "A"), and
WHEREAS, It is in the best interest of the City to obtained the surveys
(Exhibit "A").
NOW THEREFORE, BE IT RESOLVED THAT THE CITY COMMISSION
OF THE CITY OF OPA-LOCKA, FLORIDA:
Section 1. The recitals to the preamble herein are incorporated by reference;
Section 2. The City Manager is hereby authorized to spend $1400.00 for the
two surveys that are needed to complete the Cairo Lane project.
Section 3. This Resolution shall take effect immediately upon adoption, and
is subject to the approval of the State of Florida Financial Oversight Board.

Dear Mr.Gonzalez:
Premiere Design Solutions,Inc.(PDS)is pleased to submit this proposal for the above referenced
project to the City of Opa-Locka (Client), to provide Professional Surveying services for this
project inside in the City of Opa-Locka,Miami-Dade County(County),Florida.
The project consists of the drafting of legal description and sketch for an easements to be
negotiated by the City and property owner. These services will provide the Client one document
that includes a one page legal description and sketch per property. The legal description and
sketch provided will be utilized by the City and will be included in their preparation of documents
and negotiation of easements. Services do not include any negotiation support,recording,or any
other activities necessary for actually obtaining the easements. Property is located in the East of
Cairo Lane and South NW 127th Street.
SCOPE OF SERVICES
In order to meet the Client's need for this emergency project, PDS will complete the following
tasks:
Task No. 1—Sketch and Legal Description
Based on the drawing showing the proposed water improvements prepared by the City and/or its
consultant, PDS will prepare the easement documents and drawings utilizing AutoCAD. The
drawing will be prepared utilizing the State Plane Coordinate System and will include a legal
description of the proposed easement location for each subject property. Proposed easement
location has been identified starting 230.0586LF east of existing wall and shall be 12' wide by 9'
deep. A three page document will be created including a cover page,the sketch drawing showing
the easement and a legal description page. Drawings will follow our QA/QC procedures,and will
be reviewed, signed and sealed by a Florida registered Professional Land Surveyor. Documents
will be provided in hard copies (8.5"x11") 3 copies, in AutoCAD format and in PDF, submitted
via email.

2. This proposal does not include underground utility investigation.
3. No permitting or recording services are included.
4. No easement negotiation services are included.
5. No field work is included.
6. Services that are not mentioned as part of the exclusions and are not described in the scope
of services are not included.
If requested by the Client, PDS can provide a proposal for the items not included in the scope of
services.
Contract Terms and Agreements
1. This contract agreement is for a specific purpose in a lump sum basis. Additional work
performed under the scope of services will be billed on a time-spent basis per our schedule
of fees and as approved by the client in writing.
2. Invoices are due net thirty(30)days from invoice date.
3. Any invoices past due more than 30 days will accrue a 1.5%interest per month fee.

Easement property is located at 13480 Cairo Lane, identified with Folio#08-2128-003-0540.
SCOPE OF SERVICES
In order to meet the Client's need for this emergency project, PDS will complete the following
tasks:
Task No. 1 —Sketch and Legal Description
Based on the drawing showing the need of the proposed easement prepared by the City and/or its
consultant, PDS will prepare the easement documents and drawings utilizing AutoCAD. The
drawing will be prepared utilizing the State Plane Coordinate System and will include a legal
description of the proposed easement location. Proposed easement location has been identified at
the SW corner of the intersection of NE 135 Street and Cairo Lane, in a triangular shape having a
westerly dimension of 8' and a southerly dimension of 11'. A three page document will be created
including a cover page, the sketch drawing showing the easement and a legal description page.
Drawings will follow our QA/QC procedures, and will be reviewed, signed and sealed by a
Florida registered Professional Surveyor and Mapper. Documents will be provided in hard copies
(8.5„x11„) 3 copies, in AutoCAD format and in PDF, submitted via email.
