COUNCIL CHAMBERS – 5938 KAUFFMAN AVENUE PROCEDURE TO ADDRESS CITY COUNCIL: If you wish to address the City Council, please complete a Speaker Request Form, indicating the agenda item number and submit it to the City Clerk. There will be a maximum 30-minute period for comments on any subject with a 3-minute time limit on each speaker, unless waived by Council. CONSENT CALENDAR All Consent Calendar items may be approved in a single motion as recommended unless removed for further discussion. If members of the City Council or persons in the audience wish to discuss any matters listed on the Consent Calendar, please address them at this time. Recommendation: Approve Items A through G per recommendations. A. APPROVAL OF MINUTES The City Council is requested to review and approve: 1. Minutes of the Special City Council Meeting of September 6, 2016; and 2. Minutes of the Regular City Council Meeting of September 6, 2016. Recommendation: Approve. B. APPROVAL OF PERMITS FOR USE OF THE LOS ANGELES COUNTY PARKING LOT, LOCATED AT 5939 GOLDEN WEST AVENUE, FOR THE 2016 FALL FESTIVAL AND THE 2017 CAMELLIA FESTIVAL Approval of the permits for the use of the Los Angeles County parking lot will allow the City to use the parking lot adjacent to the library at 5939 Golden West Avenue for various activities during the 2016 Fall Festival and 2017 Camellia Festival. Recommendation: Approve the permits for use of the Los Angeles County Library parking lot for both the 2016 Fall Festival and the 2017 Camellia Festival and authorize staff to execute. C. FINAL SUBDIVISION MAP APPROVAL FOR PARCEL MAP NO. 72700, A FOUR-UNIT CONDOMINIUM PROJECT AT 6210, 6216 ROSEMEAD BOULEVARD The tentative map for the residential condominium development at 6210, 6216 Rosemead Boulevard was approved in 2015. The applicant has fulfilled the conditions required for the final map. Recommendation: 1. Approve the final subdivision map for Parcel Map No. 72700 and dedications indicated on the map; and City Council Agenda September 20, 2016 Page 3 of 6 2. Authorize the City Clerk to certify Parcel Map No. 72700. D. FINAL MAP APPROVAL FOR PARCEL MAP NO. 73034, A LOT-SPLIT SUBDIVISION AT 5715 ROWLAND AVENUE The tentative parcel map for the proposed subdivision was approved in 2015. The property owner has made the deposit for the right-of-way improvement and has signed the Subdivision Improvement Agreement. Recommendation: 1. Receive the Subdivision Improvement Agreement; 2. Approve Parcel Map No. 73034 and dedications indicated on the map; 3. Authorize the City Manager to execute the Subdivision Improvement Agreement; and 4. Authorize the City Clerk to certify Parcel map No. 73034. E. AWARD OF PUBLIC WORKS CONTRACT TO PAVEMENT COATINGS CO. FOR FISCAL YEAR 2016-17 PAVEMENT REHABILITATION PROJECT (CIP NO. P13-02) The Fiscal Year 2016-17 City Budget includes an allocation of $1.5 million in Measure R transportation funds for street pavement rehabilitation pursuant to the City’s Pavement Management Program. This project will utilize that funding to resurface about 2 million square feet of pavement. Recommendation: 1. Award a Public Works Contract to Pavement Coatings Co. as the lowest responsible bidder for the Fiscal Year 2016-17 Pavement Rehabilitation Project; 2. Authorize the City Manager to execute the Contract in an amount of $661,850; and 2. Authorize the City Manager to approve work items from the Additive Bid Schedule for additional pavement rehabilitation areas (approximately 60,000 square feet); and local curb, gutter, and sidewalk repairs within the project areas with an amount not to exceed $200,000; with a total construction cost no to exceed $1,000,000, including contingency City Council Agenda September 20, 2016 Page 4 of 6 (Additive Bid Items A.3.1, A.3.2, A.4.1, A.5.1, A.5.2, A.8, A.9, and A.10). F. VISA CARD REPORT The City Council is requested to review, receive and file the Visa Card Report. Recommendation: Receive and file report. G. ADOPTION OF RESOLUTION NO. 16-5206 APPROVING PAYMENT OF BILLS FOR FISCAL YEAR 2016-17 The City Council is requested to adopt Resolution No. 16-5206 authorizing the payment of bills.
