Cone Constructors, Inc. is pleased to respond to your solicitation for proposals for Design,
Permitting, and Construction of the "West Mashta Drive Bridge project" to be constructed
within the Village of Key Biscayne.
We feel that we have put together an extremely knowledgeable and experienced Design -Build
Team consisting of all well-known and respected designers and contractors.
You will note after reviewing our design plans that we are proposing to maintain two-way
traffic at all times during construction, instead of a single -lane traffic approach required.
It is our experience on previous projects that single -lane options do not provide adequate
safety.
We offer our proposal and qualifications for your review and are available to answer any
questions you may have or meet with you at any time.

The Principal shall give a Performance Bond and a Payment Bond with good and sufficient surety, as required by the
Contract Documents, for the faithful performance and proper fulfillment of such Contract and for the prompt payment
of all persons furnishing labor or materials in connection therewith Having met these obligations shall tender this Bond
void and of no effect or in the event of withdrawal of said Proposal within the period specified, or in the event of failure
to comply with the Contract Documents, or in the event of failure to enter into such Contract and give such Bonds and
evidence of insurance within the time specified. if the Principal shall pay the Village the difference between the amounts
specified in said Proposal and the amount for which the Village may procure the required work and supplies, provided
the latter amount be in excess of the former, then the above obligations shall be void and of no effect; otherwise, to
remain in full force and virtue.

The information solicited from Proposers by the questions on pages PQ -
1 - PQ -18 of this document is to assist The Village of Key Biscayne in
the evaluation of the competence and responsibility of a Proposer as
explained in the Request for Acquisition. Responses should be brief and
direct, but more importantly, complete. If the space provided in the
questionnaire for any response is inadequate, such response may be made
on separate sheet adequate and cross referenced to the item being
answered. All responses, whether on the questionnaire or on
attachments, shall be clearly printed or typewritten in the English
language.
Copies of existing organization chart, operating procedures and similar
material may be provided as addenda to a specific response if such
material is relevant. Public relations, sales promotional and similar
types of material should not be submitted. If a Proposer is successful,
the award of Contract by the Village shall in no way imply approval or
acceptance of all or any part of the material and information submitted
in or with this questionnaire, nor shall it form the basis of relief of
any of the requirements of the Contract as executed subsequent to award.

SAFETY PROGRAM
Jobsite safety for all employees and subcontractors is of primary importance to Cone
Constructors, Inc. The Company's excellent safety record results from the development and
implementation of a comprehensive safety policy and program.
An important distinction of the Cone Constructors, Inc. safety program is that the full
responsibility and authority for the jobsite safety rests with the Project Superintendent.
Key elements in the implementation and control of the safety program are:
• Written policies and procedures documented in the construction Policy Manual
• Training and education conducted for all levels of operating personnel, including
classroom training and weekly on -site safety meetings.
• Pre -employment checks of employees worker's compensation records and physical
condition to assure that they are physically able to perform their assignments in a safe
manner.
• Indoctrination of all new employees, issuance of a safety handbook and requirement
of written receipt of safety instructions.
• Daily safety inspections, with a written checklist report done weekly, plus inspections
by our worker's compensation loss prevention a\engineers which objectively
document the site's safety efforts.
• Safety procedures discussed in the preconstruction meeting for each building trade
prior to commencement on site.
• An aggressive Drug -Free Workplace policy for each jobsite.

DRUG FREE WORKPLACE POLICY
Cone Constructors, Inc. is committed, in its responsibility as a corporate citizen, to maintain
a safe working environment and to comply with the Drug -Free Workplace Act of 1988. All
company facilities, whether m an office or on a jobsite, are declared drug -free workplaces.
• Drug Awareness Program
Cone Constructors, Inc. distributes a copy of the policy to all employees along
with information regarding the dangers of drugs and alcohol, and how to get
assistance with drug and alcohol problems.
• Prohibitions
Under this policy employees may not use, possess, manufacture, distribute,
dispense, sell or be under the influence of illegal drugs or controlled
substances on company premises or while on company business, or in
company supplied vehicles. Each employee must sign a statement to abide by

• Penalties for Violation of the Policy
Penalties for violation of the policy vary from reprimand, mandatory

Staff Engineer for the design of 0.25 miles of two lane municipal roadway
reconstruction. The project included drainage improvements, landscaping,
pavement marking and signing plans.
* Cairo Lane Street Improvements, 1992, Opa-locka, Florida
Project Engineer for the design of approximately 0.5 miles of roadway resurfacing
for a three lane section. This road is presently servicing a heavy industrial area.
The project is being funded through a special assessment and include; roadway
design, pavement markings and ground signage, drainage design, landscaping, and
provisions for water and sanitary sewer services.
* S.W. 40th Street from US -1 to S.W. 27th Avenue Miami, Florida for Dade County
Public Works Department, 1992
Staff Engineer on approximately 0.8 miles of roadway widening, involved in utility
adjustments portion of the project.

Project Manager and Senior Engineer for the preparation of bridge plans for two,
access ramp interchanges. The South access ramps consist of approximately 1200
linear feet of two lane bridge structure with six foot shoulders. The design of the
ramps include the design of a 120 foot span tie-in along the alignment of the main
bridge. The access ramps themselves include almost 90 degree curves with a 56
foot radius. These bridges will provide access from a proposed high level bridge
over Jewfish Creek to the adjacent existing causeway. Also included the design of
a north ramp interchange consisting of 1800 linear feet of roadway embankment
using geogrids over the soft soils.
* Bridge Creek Bridge on CR -10A in Holmes County Florida, FDOT District III, 1993

piers, pier caps and prestressed piling. The bridges required widening towards the
median as well as to the exterior. This widening required the use of modified
AASHTO Type III girders due to the clearance constraints over US -92.
* Historical Relighting of the Venetian Causeway Bridges - 1994/1995, Florida
Department of Transportation - District VI.
Senior Project Engineer responsible for the historical relighting of the Venetian
Causeway Bridges. This project included the development of special street lighting
poles to recreate the original lighting fixtures when the bridges were completed in
1926. In addition to the lighting fixtures, this project required special "step-down"
transformers to comply with the current National Electric Code.

miles. The project consisted of full geometrics, drainage design, signing and

crossed two canals which required to be culverted.
* Pine Island Road Bridge Culvert Structure over the Central Broward Drainage Districts
Canal No. N-12, 1987.
Design of culvert widening for a 84 inch diameter bridge culvert. The existing
roadway section consisted of a two lane road over a 170 foot Culvert bridge. The
culvert was extended on both sides an additional 130 feet to the South and 165 feet
to the North due to the skew in the Roadway.
* Oakland Park Boulevard Bascule Bridge Rehabilitation, FDOT District IV, 1994.
Senior Design Engineer for the rehabilitation of the Bascule bridge which consisted
of a seven span structure. The rehabilitated components included restoration of the
bulkhead seawall including new tiebacks, repairs to the sloped abutments, repairs

Mr. Bunnell was born in Miami, Florida in 1943 and has been a residence of Miami with the
exception of High School in Naples, Florida. Mr. Bunnell's family has been involved in the Heavy
and Marine Construction business in South Florida since 1911. His grandfather was involved in many
projects with Henry Flagler and Carl Fisher. His father, George A. Bunnell, did foundations, Marine
Construction and swimming pools, including work on many Miami Landmarks such as the
Fontainebleau Hotel, Eden Roc Hotel, Orange Bowl and many others.
Mr. Bunnell has been engaged in the same business in Miami for over 25 years and he has also
constructed or been involved in the construction of many Miami Landmarks, as well:

Bunnell Foundation was a major contractor on Metrorail in the early 80's and worked on all phases of

Mr. Bunnell received his State General Contractors License in 1973, and also holds a Dade County
General Engineering Contracting License.
Mr. Bunnell resides in Coconut Grove, and is a member of many organizations including:
