AN ORDINANCE OF THE CITY OF JEFFERSON, MISSOURI, AUTHORIZING
UNIMERICA AS GROUP LONG-TERM DISABILITY INSURANCE CARRIER FOR
EMPLOYEES OF THE CITY OF JEFFERSON.
BE IT ENACTED BY THE COUNCIL OF THE CITY OF JEFFERSON, MISSOURI, AS
FOLLOWS:
Section 1. The Mayor and City Clerk are hereby authorized and directed to
execute documents authorizing UNIMERICA, a wholly owned subsidiary of United
Healthcare, as the city's Group Long-Term Disability Insurance Carrier.
Section 2. The Certificate of Insurance will be Issued by the company upon
approval of this ordinance.
Section 3. This Ordinance shall be In full force and effect on January 1, 2007.
