Re: Calusa Park (the "Park") Curative, Inc. COVID-19 Testing Van (the Testing Van")
Dear Ms. Wilson-Ciranna,
The Village is excited about the opportunity to extend the duration of the Testing Van located
within the Park. The Village agrees to grant Curative, Inc., a revocable, non-exclusive license to locate the
Testing Van in the Park until January 31, 2021, subject to the same terms and conditions of the Agreement,
dated December 14, 2020, between the Village and Curative, Inc. Please countersign this letter
acknowledging that you agree to the terms and conditions provided above. If you have any questions or
concerns, please do not hesitate to contact myself or my designee, Dr. Roland Samimy, VKB Chief

Roland: Attached please find the fully executed extension dated January 5 for the extension through January 31, 2021
for the Curative testing van. I just received this from Curative's attorney. Please retain a copy with the license
agreement and ensure the Clerk's office has it as well. The extension through June 30, 2021 is pending execution by
Curative.

Scott, hope you're well. The Village of Key Biscayne, Florida has approved another extension for the testing van at Calusa
Park through June 30, 2021. A copy is attached for your review and Curative's execution if agreeable. In addition, I don't

think we ever received the last extension to extend Curative's license for the testing van at Calusa Park through January
31, 2021. Can you please send us a copy? Let me know if you have any questions on either extension.
