Dear Mr. Thrasher:
Please accept this letter as the proposal for the preparation of the Town of Gulf Stream's Second Term,
Eighth Year Annual NPDES report. The Year 8 permit term runs from October 1, 2009 through
September 30, 2010.
Mathews Consulting, Inc. will provide a listing of the required operation and maintenance data for the
permit term to be collected by the Town, meet with the Town staff to review the data and discuss Town
operational procedures with regards to the NPDES program, and collate and prepare the Town's report for
inclusion in the Palm Beach County MS4 report to the State Department of Environmental Protection.
The deadline for the report to be provided to the Lead Permittee is anticipated to be December 31, 2010.
The lump sum fee for this work is $3,600.

This letter is a listing of the information we need for you to compile in order for Mathews Consulting to
prepare the NPDES Annual Report for the 2"d Permit Term — 7d' Year. The information should be for the
permit term from October 1, 2008 through September 30, 2009. The draft report is due to Mock Roos for
review by December 1, so if you could have the information together by November 17", I would
appreciate it.

Third Term Joint Permit Renewal
Dear Mr. Thrasher:
Year 5 of the five year permit term runs from October 1, 2006 through September 30, 2006. The
group is therefore submitting for our next five year permit renewal application, with a planned
submittal by the end of December 2006. (Each year an annual report is due to the FDEP but this
is for the overall permit held by the co -permittees, including the Town).
Mathews Consulting proposes to complete the following tasks on behalf of the Town:
1. Review and comment on Part III. - Schedule for Implemental and Compliance
— Implementation of Stormwater Management Programs
2. Review and comment on Table II. La - Suggested Inspection and Maintenance
Schedule of Activities
3. Review and propose necessary changes to the Town of Gulf Stream's Tailored
Annual Report Form
4. Review and comment on the draft re-application of the permit.
We will bill on an hourly basis with an anticipated fee not -to -exceed $1,500.
