Dear Mr. Thrasher:
We look forward to providing our services to your employees for the next
contract year of October 1, 2002 through September 30, 2003. Per our
previous conversation, there will be no increase in the fee of $65.00 per
session. With the addition of the legal and financial services, however,
the administrative fee has been changed to $33.75 per month. With
your approval, all terms and conditions of your EAP Agreement will
remain in effect. Please sign this renewal below, retaining one copy for
your files, and return this original to the EAP Department, 471 Spencer
Drive, West Palm Beach, FL 33409. Along with this renewal, please also
be kind enough to include an updated listing of your employees.
Please note our policy regarding cancelled appointments. Failure to
provide 24 - hour notice of cancellation of a scheduled appointment or
failure to show for a scheduled appointment, will result in forfeiture of one
of the employee's contracted sessions.
Also, we would like to advise you that we are reducing the price of our
EAP workshop offerings to $125 per hour. These include a broad array of
workplace and wellness topics of contemporary interest. Our staff is
available to discuss the needs of your organization. Please feel free to
contact us at your convenience. We are happy to be your EAP provider,
and look forward to providing you with the highest quality service
possible
