Section 1. Term of Agreement Subject to the provisions of Section 20 "Termination of Agreement" of this Agreement, the Term of this Agreement is until Marsh July 31, 2017, commencing on the date first ascribed above. Section 4. Compensation and Method of Payment (a) Subject to any limitations set forth in this Agreement, City agrees to pay Service Provider the amounts specified in Exhibit `B" "Compensation" and made a part of this Agreement by reference. The total compensation, including reimbursement for actual expenses, shall not exceed «..fetifhaus._a ae"a-s ($24,000) fifty five thousand dollars ($55,000), unless additional compensation is approved in writing in accordance with Section 26 "Administration and Implementation" or Section 28 "Amendment" of this Agreement. Exhibit "A": Scope of Services 3. PfeliminaFy Relocation Assistance Services, i.e., • Provide occupant households with the following documentation when appropriate: general information notice; notice of eligibility; initial and updated letters of entitlement; referrals to replacement sites; and 90 -day, 60 -day and 30 -day notices to vacate (to be signed by City). Secure necessary documentation from occupant households and other sources to determine household income, occupant status and replacement site needs. This information will be used to determine the maximum replacement housing payment available for each household. First Amendment to Professional Services Agreement Associated Right of Way Services Page 2 of 2 Research the area housing market (e.g., through area brokers, classified ads, driving surveys, database services, etc.) and develop a housing valuation sHidy studies (as needed) to determine the cost of comparable replacement housing for each type of housing need. (Ff4l+e additiatial ealeulations afid analyses.) A detailed work scope and tentative ifflplefflefi�aizieft • Assist occupant households with searching for and securing available replacement housing. This includes: compiling available housing information; touring available sites with household occupants; assisting with rental applications or escrow due diligence; and providing documentation to prospective landlords and realtors. • Facilitate the payment of all claim forms for occupant households, and secure necessary documentation and signatures for claims. This includes preparing and processing said forms in conjunction with City, and delivering payments (on behalf of City) to occupant households. • Establish and maintain a detailed relocation file for each occupant household for documentation purposes, Each file, to provide to City at end of contract, will include a relocation diary to track Service Provider's contact with occupant households. • Inspect replacement dwellings on behalf of City to determine whether replacement properties for occupant households meet decent, safe and sanitary (DS&S) standards as defined in State relocation regulations. • Coordinate pre- and post -relocation efforts, which may include but not be limited to: securing cost estimates from professional movers; coordinating the return of security deposits and other payments; inspecting City's properties to determine vacancy; and finalizing Abandonment Statements and any other necessary paperwork. Exhibit `B": Compensation All provided Services shall not exceed a cumulative total of $24,000 $55,000 and must be completed by A4areh July 31, 2017. To accept this Amendment, please complete the following and provide three signed copies for execution to Jillian Nunez, Deputy City Clerk, at the address above.
